Specialty Telehealth Services

  • Time-Based Rate:

    The minimum appointment duration is 30 minutes at a rate of $185. Additional time beyond 30 minutes is billed on a prorated basis at $20 per additional 5-minute increment.

    Dr. Barnum will review patient history, discuss main concern, answer any related questions and provide recommendations / next steps guidance.

  • Time-Based Rate:

    The minimum appointment duration is 15 minutes at a rate of $135. Additional time beyond 15 minutes is billed on a prorated basis at $15 per additional 5-minute increment.

    Dr. Barnum will discuss / complete request for one main concern per established patient visit.

  • For those seeking additional medical communication support & advocacy:

    New Patient Fee: $425

    Established Patient Fee: $375

    **Includes 3 calls related to concern (final price will vary if additional calls are needed for more complex cases).

    Call #1: New Patients: Initial Virtual Visit (30 min); Established Patients: Single Virtual Visit (15 min) to discuss medical situation that needs support/advocacy.

    Call #2: After evaluating your case, Dr. Barnum will reach out & communicate directly with your treating physician or provider to clarify unanswered questions and help move your care forward.

    Call #3: Dr. Barnum will give you a phone call (15 min) to follow-up / update you on the matter & provide next steps guidance.

We offer a range of case-specific virtual services for anyone residing in the state of Florida. Please review the lists of cases we do and do not cover below.

Services that we DO cover (click arrow to read more):

  • With consideration of prescription after evaluating (e.g., short-term antibiotics, rescue inhaler, uncomplicated UTI, skin condition, nausea medication)

    • If relevant to the case, the patient may upload photos to their portal prior to the visit

    • Prescribing decisions are made at the physician’s discretion based on clinical appropriateness

  • *Maximum 30-day supply – for patients in urgent need of their med refill but unable to get in with their provider.  

    • When filling out your intake form, it must upload med list, last refill date, condition the medication is treating, and preferred pharmacy

    • Dr. Barnum will review your intake/uploads prior to the telehealth visit

    • If certain information is missing, we will reach out to you prior to session

    • If unable to obtain information in time prior to session, visit will be postponed

    • After this visit, the patient must be evaluated by PCP to be considered for future refills after the first visit

    • For prescriptions without insurance, Dr. Barnum recommends Walgreens Rx Savings Finder and can text/email a coupon code for you to show the pharmacist

    • Each refill requires a separate telehealth visit

  • *Must be approved by Dr. Barnum before scheduling visit. After the virtual visit, the requested note/letter will be emailed to you with Dr. Barnum’s physician’s signature.

  • If deemed appropriate after your virtual visit, Dr. Barnum will send or eFax the requested referral to the recommended provider (e.g., Physician, Physical Therapist)

  • Completion of Form HSMV 83039: (83039.pdf) – patient must fill out the initial part of the form first, then scan and upload to our portal. 

    NOTE: any additional justifying documentation you can add to the portal will increase likelihood of approval

  • For those seeking decision support, conflicting medical opinions, fear-based recommendations, or other overwhelming scenarios.

  • Includes Medication Counseling & Treatment Recommendations – patient sends full medication list, Dr. Barnum reviews in advance, and makes recommendations (e.g., continuing or discontinuing medications, medication interactions, redundancies, risk/benefit analysis, medication side effects, coordination with PCP)

    NOTE: the patient should then follow-up with their treating physician regarding final medication changes

  • Comprehensive discussion of your previous lab findings with recommendations of next steps/providers

  • Discussion of the medical topic(s) of your choosing with the allotted timeframe (pertaining to Family Medicine/Neuromusculoskeletal Medicine; e.g., diet, exercise, sleep hygiene, self-mobilization, postural retraining, breathing mechanics, nervous system regulation)

  • Discussion of previous findings, explanation and clarity regarding another provider’s findings, and follow-up recommendations

  • Education on your imaging reports (e.g., MRI/X-ray), biomechanical interpretation, next-step options, alternative treatments other than surgery

    *Dr. Barnum does not read images, but can interpret imaging report findings.

  • In-depth discussion of chronic medical conditions, review of detailed medical records uploaded by patient, and discussion of recommended next steps


  • A detailed discussion of your pain status with next-step recommendations (Non-Procedural).

  • A physician-led advocacy service for patients who feel unheard or are seeking additional medical communication support.

    After evaluating your case, Dr. Barnum will communicate directly with your treating physician to clarify unanswered questions and help move your care forward. Depending on the complexity of the case, this service may require multiple telehealth visits. 

    • Dr. Barnum highly recommends obtaining digital office visit notes in advance to upload to our portal for review prior to the telehealth visit.

    • Inability to provide records for certain cases may result in postponement of visit until the necessary information is obtained.

    • This service focuses on communication, clarification, and advocacy. It does not involve legal representation, medical decision-making for other providers, or interference with ongoing care.

  • For adult children, caregivers, out-of-state family members needing a medical opinion or guidance regarding their family member residing in Florida. This will be an opportunity to explain rehab programs, prognosis clarification, discharge planning advice, etc.

    *Patient being discussed must be residing in Florida at the time of the virtual visit.

  • Education on more holistic/alternative adjunctive approaches to treatment for your condition to reduce the need for medication

Services that we do NOT cover (click arrow to read more):

  • We Do Not Provide:

    • Emergency Care (e.g., acute chest pain, shortness of breath, stroke symptoms, severe neurologic deficits)

    • Life-threatening or unstable medical conditions

    • Mental health crisis care, including suicidal ideation requiring immediate intervention

    • Pediatric emergencies or acute medical distress

    Note: Patients with urgent or emergent symptoms should seek immediate in-person or emergency care.

  • We Do Not Provide:

    • Primary Care Provider (PCP) services

    • Long-term continuity of care

    • Chronic disease management

    • Hospice or palliative care

    • Preventive screening or annual physical examinations

  • We Do Not Provide

    • Medication refills longer than 30 days

    • Controlled substances

    • Benzodiazepines, stimulants, or opioids

    • Psychiatric medications (unless stable and well-documented)

    • Medications requiring laboratory monitoring unless recent labs are verified

    • Initiation of GLP-1 medications (e.g., Ozempic, Wegovy, Mounjaro, Zepbound)

    • Initiation of hormone replacement therapy (e.g., estrogen, testosterone)

    • Medical marijuana certifications

  • We Do Not Provide:

    • Insurance billing

    • Prior authorizations

    • Disability ratings, impairment determinations, or disability certifications

    • Medical opinions or documentation for legal proceedings

    • Workers’ compensation evaluations

    • DOT paperwork

    • Title 19 Medicaid forms

    • Forms requiring an in-person physical examination

  • We Do Not Provide:

    • Ordering imaging studies (must be done in-office at Dr. Barnum’s Neuromusculoskeletal clinic)

    • Initiating laboratory orders

    • Performing physical examinations

    • Fitness-for-duty, pre-employment, or occupational clearance exams

  • We Do Not Offer:

    • Insurance-based models

    • Subscription or membership-based care

    • Concierge primary care models
